Terms of Service

These Terms of Service ("Terms") cover CICO Behavior Analyzer, the web app at cico.helixforms.com and its demo (the "Service"), provided by Helix Forms LLC ("we," "us," or "our"). By using the Service or buying a license, you agree to these Terms. If you use the Service for a school or district, you confirm that you are allowed to accept these Terms for it.
1. What the Service is
CICO Behavior Analyzer helps school staff score Check-In / Check-Out point sheets, keep intervention plans, analyze progress, and print or email reports. It runs in your web browser and works with a Google Sheet in your own Google account (one spreadsheet per building). We do not host or store the information you enter; see our Privacy Policy.
The demo at cico.helixforms.com/demo uses made-up students, requires no sign-in, and saves nothing.
2. Who may use it
- The Service is for adult school staff and professionals (for example school psychologists, counselors, interventionists, teachers, and administrators). It is not meant to be used by students.
- You need a Google account. Licenses are for school Google accounts; personal accounts may use the free trial.
- You are responsible for your Google account, for who you share your building's spreadsheet with, and for the access you give them in Google Drive.
3. Free trial
Each Google account gets one free 14-day trial with every feature turned on, starting the first time that account signs in. After the trial, the Service is read-only for that account until it is covered by a license (section 5).
4. Licenses
- Individual: one person, identified by one school email address.
- Building: everyone who signs in with an account at one school's email domain, for use in one building.
- District: everyone at the district's email domain, priced per building, for the number of buildings purchased.
Licenses are based on trust: please buy the license that matches how the Service is actually used (for example, a Building license for each building that uses it, or a District license covering all of them). A license may not be resold, transferred to another school or district, or shared with people it doesn't cover.
5. Payment, term, and renewal
- Prices are shown on our website at the time of purchase and are charged in U.S. dollars through Stripe, our payment processor. We can also invoice a school or district by purchase order on request.
- A license lasts one year from the day we turn it on, which is usually within one school day of payment. Licenses do not renew automatically: we remind you in the app starting 30 days before the end date, and you renew by purchasing again.
- After the end date the Service keeps working for 14 more days while a renewal goes through. After that it turns read-only: you can still view, analyze, print, copy, and email everything, but nothing new is saved until the license is renewed.
- We may change prices for future purchases and renewals. A change never affects a license you have already paid for.
6. Refunds
If the Service isn't working for you, email us within 30 days of your purchase for a full refund. After 30 days, payments are not refundable, except where the law requires otherwise.
7. Your data
- Everything you enter (students, plans, scores, notes, contact emails) stays in your spreadsheet in your Google account. You own it, and you can open, edit, copy, or delete it in Google Sheets at any time, with or without a license.
- You are responsible for your students' records: for having the right to keep and share them, for following your school's policies and laws such as FERPA, and for keeping the spreadsheet shared only with the right people. Google Drive's version history is your backup; we keep no copy.
- Email features open a draft in your own Gmail for you to review and send. The Service never sends email on its own.
8. Acceptable use
Don't use the Service to break the law or anyone's rights; don't try to get around the license check, copy or resell the Service, or interfere with how it runs; and don't use it to store information unrelated to supporting students' behavior and learning.
9. Professional judgment
The Service's statuses, trends, projections, and suggestions (such as "Review the Plan" or "Ready to Fade") are aids to your team's decisions, not the decisions themselves. Decisions about students' supports, placement, and services remain your team's professional responsibility.
10. Changes and availability
We improve the Service over time and may add, change, or remove features. We aim to keep it available, but it depends on Google's services and your internet connection, and it may occasionally be unavailable. If our license check can't be reached, the app keeps working normally. If we ever discontinue the Service, we will give at least 60 days' notice and refund the unused part of any license; your spreadsheets stay in your Google account either way.
11. Our intellectual property
The Service, its design, and its code belong to Helix Forms LLC. A license gives you the right to use the Service during its term; it doesn't transfer ownership of the Service. Printouts and reports you make with it are yours to use.
12. Disclaimer
The Service is provided "as is" and "as available." To the extent the law allows, we disclaim all warranties, express or implied, including warranties of merchantability, fitness for a particular purpose, and non-infringement, and we don't promise that the Service will be error-free or uninterrupted.
13. Limitation of liability
To the extent the law allows, Helix Forms LLC will not be liable for indirect, incidental, special, consequential, or punitive damages, or for lost data or profits. Our total liability for any claim about the Service is limited to the amount you paid us for it in the 12 months before the claim.
14. Ending use
You can stop using the Service at any time, and remove its access to your Google account at myaccount.google.com/connections. We may suspend access for anyone who seriously or repeatedly breaks these Terms, after notice when practical.
15. Governing law
These Terms are governed by the laws of the State of Iowa, without regard to its conflict-of-law rules. If a public school or district is required by law to follow different terms on governing law, those requirements apply to it.
16. Changes to these Terms
We may update these Terms. Changes are posted on this page with a new "Last updated" date. Changes don't apply retroactively to a license already paid for, and continuing to use the Service after a change means you accept it.
